A highly trained interdisciplinary Hospice Team partners with the patient, family and friends to create an individualized plan of care which encourages the patient to live his or her life to the fullest while receiving specialized support and services.Family's first experience with hospice, we were all extremely impressed with the program and were so thankful for such a wonderful program.
