Sholom Home Care provides client-centered care by a highly trained team of nurses, home health aides, companions and homemakers. Our goal is to help each client reach and maintain his/her highest level of function and independence. Staff is available 24 hours a day, seven days a week.
Our registered nurse will assess your health history, confer with you and your family and coordinate with your physician to develop an appropriate plan of care to meet your individualized needs.
A home health aide is available in our building 24-hours a day, seven days a week. In addition, a nurse is on call at all times. Our staff is committed to meeting your special needs and to keeping you safe in your apartment.
